Sequential Intraoperative Evaluation of Rotational Stability of Cementless Stem in Total Hip Arthroplasty: A Broach-Based Study

In cementless total hip arthroplasty, the rotational stability of stem is generally confirmed in a one-time manual evaluation by surgeon. This study was performed to evaluate sequential intraoperative changes using torque wrench. Primary arthroplasty on 52 consecutive hips single wedge design. Intraoperative uniformly Evaluations were immediately after broach insertion and before final (after placement acetabular cup). Immediately broach, checked be fully stabilized wrench all cases, maintained 17 (33%) stem. Among showing instability, 11 35 (31%) broaches upsized, while remaining 24 achieved stabilization through deeper broach. conclusion, not necessarily during surgery, indicating that may need at multiple time points intraoperatively.
